I hereby grant The MMBC Continuum, its departments and related subsidiaries, its representatives, and authorized partners the irrevocable right and permission to capture, use, reproduce, edit, distribute, and display my image, likeness, voice, and statements in photographs, video recordings, audio recordings, and any other media formats (collectively, "Media") for promotional, educational, marketing, and branding purposes.

I understand and agree that:

1. Use of Media – The Media may be used in printed publications, digital platforms, social media, websites, press releases, advertisements, and any other promotional materials deemed appropriate.
2. No Compensation – I will not receive any monetary compensation for the use of the Media, nor will I have any ownership rights or control over how it is used.
3. Release of Claims – I waive any right to inspect or approve the final version of the Media and release The MMBC Continuum, its officers, employees, contractors, affiliates, and agents from any claims, demands, or liabilities related to its use.
4. Use – My name, likeness, or personal information will not be used in a defamatory or misleading manner.
5. Duration & Revocation – This release is effective indefinitely unless revoked in writing by sending a request. However, I acknowledge that previously published Media cannot be retracted.

By checking the box and moving forward with registration, I confirm that I am at least 18 years of age (or have parental/guardian consent if under 18), have read and understand this agreement, and voluntarily grant permission as stated above.

All text, graphics and other original content on The MMBC Continuum website (www.mmbc-memphis.org), in MMBC Continuum/MMC-TADP, Inc. training materials, and/or in MMBC Continuum/MMC-TADP, Inc. presentations, whether written or oral (collectively "Content"), are protected by the copyright laws of the United States and are the property of the MMBC Continuum, or as otherwise credited.

You may not copy, reproduce, distribute, publish, display, perform, record, transcribe, modify, create derivative works from, transmit, sell, offer for sale, license, or use such Content in any other means or manner, without the prior express written consent of The MMBC Continuum. You may not alter or remove any copyright or other notice from the Content.
